Application Process

Can I Apply Online?

In order for you to apply online for a Medical Marijuana Id card your physician must be registered in the system to accept online applications. Please contact your physician and ask if they have registered in the online application program.

Your Physician IS registered to accept online applications, AND you have seen your Physician within the past 90 days.

Your Physician IS NOT registered to accept online applications; please print the paper application and take it to your Physician.

Patients and Caregivers Application Process

Let us guide you through the process of applying for a Medical Marijuana ID card. Whether a caregiver or a patient, you will find step-by-step instructions to help you navigate the application process.

Patient Applications

In order to qualify for a standard Medical Marijuana card, you must meetthe following qualifications:

  Be 18 years of age or older or be a pediatric patient with parental consent.
  Have a Health Care Practitioner Certification from a Delaware licensed MD, DO, APRN or PA.
  Be a Delaware resident with proof of residency. (DE Driver's License or DE State ID)

Patients 65 and Older

May self-certifytheir qualification for a medical marijuana ID card without written certification from a health care provider. You are still required to complete and submit the patient application.The only difference is your healthcare provider does not have to sign the application.

Applications may be completed online (If you qualify) or by paper application.

  Health Care Practitioner Certification

The first step to getting a medical marijuana registry card in Delaware is to meet with your health care practitioner about medical marijuana.

Your health care practitioner MUST be licensed to practice in Delaware and be a MD (Medical Doctor), DO (Doctor of Osteopathic medicine), APRN (Advanced Practice RN), or PA (Physican Assistant).

Depending on if you apply online or via paper application, your health care practitioner will complete a health care practitioner certification if he or she feels that you will benefit from the use of medical marijuana.

  Patient Application

Complete the patient application, or pediatric patient application, in its entirety.

You can apply online, or print a paper copy of the application.

If you will require a caregiver, ensure that the caregiver completes a caregiver application. Information on how a caregiver can apply can be found here.

  Non-refundable Application Fee

The non-refundable application fee is $50 for 1 year; $75 for 2 years; and $100 for 3 years. If you apply online, there will be a section for payment options. If you are mailing your application, please include a check or money order for the selected application fee, payable to the State of Delaware.

  Proof of Age and Residency

You, or parents or legal guardian for pediatric patients, must upload a copy of your Delaware issued driver’s license or State issued ID with your application.

If you are mailing your application, send a clear photocopy of your Delaware issued driver’s license or State issued ID.

Caregiver Qualifications

In order to qualify for a Designated Caregiver card to legally purchase medical marijuana for a qualified patient, you must meetthe following qualifications:

  Be 21 years of age or be the parent or legal guardian of a minor child.
  Have not been convicted of an excluded felony offense.
  Have agreed to assist with a patient's medical use of marijuana.
  Be a Delaware resident with proof of residency. (DE Driver’s License or DE State ID)

Applications may be completed online (if you qualify) or by paper application.

Complete the caregiver application in its entirety. You will need the patient’s 10-digit registry number to apply.

  Non-refundable Application Fee

If you apply online, there will be a section for payment options.

If you are mailing your application, please include a check or money order for the application fee, payable to the State of Delaware.


  Proof of Age and Residency

You must provide a copy of your Delaware issued driver’s license or State issued ID with your application. If you apply online, you may upload the documents at that time.
